Life Flight Network Air Membership
The Life Flight Network offers a group discount rate of $50 (discounted from $65) a year per family. To qualify for this discount rate, PANTRA must maintain 10+ households and pay with one form of payment. Therefore, if you would like to be covered, please follow the instructions below to sign up.
How to Sign Up:
To get the PANTRA group discount, you must print out the application form below, fill it out, then mail the completed form with your $50 check (made out to PANTRA) to the address below on or before April 10th, 2019.

Benefits of Membership:
Medical emergencies and accidents happen every day without warning, and venturing outside of a metropolitan area can jeopardize a patient’s chances of survival. That’s where Life Flight Network comes in. We can quickly dispatch an emergency transport team, bringing the ICU to you. When you’re a member, there’s absolutely no cost for medically necessary emergent flights.
The following are just some of the benefits you and your loved ones will receive as Life Flight Network members:
- Membership covers you, your spouse or domestic partner, and dependents you claim on your income tax return. Elderly and disabled family members living in the same household can also be covered.
- Members will not incur out-of-pocket expenses for medically necessary emergent flights if flown by Life Flight Network or a reciprocal partner, subject to their program rules.
